Registration required.
Documents to bring:
Permanent Resident Card “Green Card”
California I.D. or Driver’s License
Social Security Number
Address and dates of where you lived in the past 5 years.
Employment history for the past 5 years (name of employer, complete address, occupation/position, and dates from start to end)
Court Records for any crimes committed (even if expunged or sealed)
Information of Current Spouse (social security number, date of birth, date of marriage, date and place spouse became a citizen, and A # if permanent resident)
Information of Prior Spouse (full name, date of marriage, date marriage ended, same information of the ex-spouse of your current marriage)
Information of all your children (complete names, dates of birth, complete address, and permanent resident # if any)
Date of all trips outside the U.S for the past 5 years
USCIS Fee: check or Money Order payable to: “Department of Homeland Security.”

Verification letter from the County of Social Service that you are receiving Public Benefits
